My take on the twins!!!
The patient gowns that they're wearing are supposed to allude to the fact that they're ultimately supposed to serve as the replacement eyes for Cthulhu, and thus giving them unique clothes serve no purpose lol.
They escaped and gave themselves minor accessories like the scarf and such to try and be more unique lol
