I was browsing the wiki when it just hit me. I'm was quite honestly surprised to never have seen this mentioned before.
Strong Evidence:
– Both are characters tied to bosses in the world
– Additionally, both these bosses have ties to Cthulhu
– Both spawn naturally in the world
– Both have Voodoo Dolls unlike every other character
Weak Evidence:
– Clothier's shimmered form bares a small resemblance to the Guide
– Both have meta knowledge of previous incarnations of themselves
Arguments:
– The "talks in a familiar way" could just be a reference to the Old Man
– They could've just both been curse by the Cult of Cthulhu without a prior connection

Not a bad theory!
but who is the mother?
THE MOON LORD?!
I don’t believe the Wall of Flesh has a connection to Cthulhu, it is the “core and guardian of the world“ and its purpose is maintaining balance, which seems like the complete opposite of Cthulhu, who brought destruction upon the world. The Guide”s curse also seems like a part of his duty and not something that was applied by someone else, especially not by Cthulhu.
An interesting theory! The Clothier “talking in a familiar way” could also be a reference to the fact that a lot of the Clothier’s lines are references to Tom Hanks movies
The old man seems to be waaaaaaaaaaaaaaaaaaaaaaay older than you think he is. He has very likely been at that dungeon for hundreds of years listening to the cult
So in terraria lore there exists a thing called the order of the guide which is an cult/group who’s whole thing is about knowledge, when the guide dies to a voodoo doll the next guide that spawns can talk about how their predecessor burst into flames randomly. So it’s less having previous knowledge of past incarnations more so when a guide dies someone else from the order takes their place, it’s not like the dryad who is the only living one of her species who reincarnates when she dies. The guide is also according to red himself the wall of flesh and has no connection to cuthulu as the wall is a guardian of the world
The last point imo could be referring to the player
The Guide is the Moon Lord’s son, and the Nymph’s grandson… this could only mean one thing; the Nymph is the Moon Lord’s mother!
