You don’t need only the house, many npcs have conditions to spawn, such as:
The merchant requires you to have a certain amount of money in your inventory
The nurse requires you to use at least 1 heart crystal
The demolitionist requires you to have explosives in your inventory
Go explore a cave and collect some coins and one of them might eventually spawn :>
