are there any blocks around tha base of the soul sands? there should be two air blocks on each side iirc

Do you have a mob heads datapack installed? If so, you need to make sure that you have skulls and not heads.

Most possible I can think of is you have like a torch or something behind the soulsand, those two corners has to be completely empty

Bc the soul sand is most commonly made to be built vertically. Like the single soul sand, and then on top of that, the 3 soul sand blocks, and on top of that, the 3 skulls.

Unneed to lower the wither by 1 block. Have the same issue. It wikl still get stuck in the portal
