What I mean is a mod that adds a bigger threat, something that has to be dealt with throughout the whole game.
The wither storm is what first came to mind when thinking about this, this force of nature that destroys pretty much anything in its path and has the players moving constantly with this feeling of urgency always in the back of their mind, regardless of what they're doing.
It sounds fun to add a similar kind of threat to Terraria, but I don't really know where to start looking for a mod like this.

There’s a solar apocalypse mod based off the one from Minecraft. Other than that there’s a Majora’s Mask mod that destroys the world every 3 days. Haven’t played either one yet tho
I was gonna mention Calamity: Wrath of the Gods, but its effects only take place after you defeat the Moon Lord, at which point your entire world surface gets permanently frozen, including the oceans AND the Abyss, and all the surface music is completely gone, the snow never stops falling and the background permanently changes. All of this stops after you defeat the superbosses introduced by the mod, but the unfreezing will drain your ocean and abyss significantly.
It doesn’t quite fit what you asked for, but I’m not that deep into modded Terraria yet.
