Found this in my house somewhere, does anyone know what mod this is from? JEI says its from Minecraft but I can’t seem to find anything that says where this item comes from. Theres also no way that someone renamed this paper and did this themselves right?

Definitely need the modpack/list for something like this.
To answer your later question, the description (that bottom part) can *usually* only be modified in the NBT data. It wouldn’t say what mod it’s from as it’s just a vanilla Minecraft paper renamed and given a description.
