Short version: The Last Secret is a hidden something ConcernedApe put in the game, but has mentioned no one to his knowledge has uncovered. He said it's not in the game's code, it's obscure and irrelevant to the game itself but that it's a message hidden somewhere else. He used the game's art as an example of it being vague and difficult to find

Pathways is a painting you can find (purchase? I'm not sure) in the game. It checks all the boxes. It's obscure. It's visual (so not a mechanic coded into the game), and has no effect on the game or story. Its seemingly disconnected to everything else, and it's been there since version 1.

A couple of months ago, someone here mentioned they think the Pathways theory is the most credible because they noticed an S and a 4 in it. I had no idea what they were on about so I went to go look and I noticed a whole lot more in.

I tried my best to mark everything I can see, but the picture (taken from the Wiki) is so tiny it frazzled my outlines (sorry about that). I'll do my best to list everything clearly. Let me know if you notice a pattern here.

Note, some of things are ambiguous or backwards, so I'm just listing possibilities here. Some shapes look like repeat letters.
